Preparation/Tutoring for the Certification Exam “Sterile Processing Technician”
This Enrollment Agreement (hereinafter the “Agreement”) is entered into by and between Multyprep, Inc. located at 6 Salkin Way, Freehold, NJ, 07728, and the above-mentioned Student (hereinafter “student”). The above-listed company and student enter into an agreement under which the student will pay tuition and fees as indicated below as well as adhere to the companie’s rules and regulations.
Engagement / Licensing Disclaimer:
*Multyprep, Inc. provides preparatory /tutoring program, and guidance.
*Multyprep, Inc. does not award accredited degrees or licenses
*Multyprep, Inc. Doesn’t provide 400 hours of Clinical Practice
*Multyprep, Inc. does not guarantee employment to the student after the completion of preparatory and training.
Refund Policy:
*When requesting a refund, the student must e-mail multyprep@gmail.com and provide an explanation
as to why he/she is canceling/requesting a refund.
*Student who cancels within five (5) days of the enrollment agreement but before instruction begins
receives all monies paid.
*Refunds will be processed within two (2) business days of an approved refund request.
*After five (5) days post-signing and prior to instruction begins, a student will be liable for $250
non-refundable registration fee paid to Multyprep, Inc.
*After the first session, refunds will not be granted under any circumstances.
UNDERSTANDING APPLICATION, POLICY and PROCEDURES
*I am hereby registering to take Multyprep, Inc. HSPA-CRCST exam preparatory/tutoring program.
*I further certify that the information provided by and about me on this form and any other subsequent
documentation submitted in relation to my registration is accurate and correct.
*I understand that the information I provide to Multyprep, Inc. may be audited for verification.
*By submitting this registration application, I attest that I have been notified about the required 400 hours of hands-on experience necessary to receive the unrestricted certificate.
*I am also aware that Multyprep, Inc. does not provide the facility nor the hands-on training portion of this
requirement with no warranties implied or provided.
*I fully understand that I will need to locate an approved/licensed facility and complete the 400 hours of hands-
on experience within six months of passing the certification exam.
Non-Disclosure Agreement
This course and its materials are confidential and proprietary. It is made available to me, the student,
solely for the purpose of preparing for a certification exam in the technical area referenced in the title
of this course. I am expressly prohibited from recording, copying, reproducing, disclosing, publishing,
or transmitting these materials, in whole or in part, in any form or by any means, verbal or written,
electronic or mechanical, for any purpose.
By signing below and submitting the course application and fee, I attest that I have read,
understand and agree to abide by Multyprep, Inc. program’s policies and procedures
